Psychological therapy is the cornerstone treatment for people with BPD / EUPD generally commencing when they are sufficiently emotionally stable and motivated to engage in their recovery through behaviour change. In some instances medications may be recommended to treat specific symptoms but medication on its own is often of very limited benefit and once a person fully engages in psychological treatment, medication may be reduced over time, if not stopped entirely.

The nature of BPD / EUPD can make it difficult to initiate and maintain a trusting relationship with a therapist and sometimes the path of therapy can be stormy with periods of idealisation as well as devaluation of the therapy process but those emotions can be worked through as part of the recovery journey.

DBT is designed to treat suicidal and self-harming behaviours, often present and problematic for those with EUPD and it has an evidence base confirming its effectiveness. DBT teaches skills to help an individual with EUPD regulate their emotional turmoil, improve interpersonal relationships, tolerate distress and become more calm and effective when under stress. It requires active engagement in the therapy process. In essence DBT acts to find a balance between changing and accepting ones life experiences, beliefs and behaviours. DBT includes a weekly emotional group skills training and a weekly therapy session with an individual therapist.
